**Full-Time and Part-Time openings available!****Every other weekend shift also available!**Care Givers will need to demonstrate reliability, initiative and professionalism as well as a caring, supportive and nurturing behavior. They must provide high quality resident care in accordance with established policies and have the skills necessary to manage the resident services and hands-on activities required by the residents and the nature of the community.KEY POSITION SPECIFIC ROLES & RESPONSIBILTIESMeet with team lead at the end of each shift to inform them about resident care, resident changes, and all important resident informationInform team lead of any resident emergency situations in a timely fashion and complete proper documentation before shift end on appropriate forms; notify on-call administration, in the absence of a team leadReview and be familiar with resident service plan and history as appropriate to ensure quality care for each residentEncourage socialization among residents; review and be familiar with resident chart as a tool to learn resident's social history and interests and review with all caregivers,Review daily assignment sheets for changes at the beginning of each shiftComplete all duties on daily assignment sheet and sign-off when completedComplete time sheets daily and accuratelyAssist with the admission, discharges and transfers of residents as assignedAssist with the hands-on care of residents such as bathing, hygiene, dressing, undressing, laundry and other activities of daily livingReport any maintenance/building or safety issues/problems to front desk personnel or complete maintenance work order in the absence of front desk personnelAssist in the dining room at meal times, serving residents, busing tables, clean-up, room set-up, and room trays as assignedFollow all Quality Standards and Core ValuesAnswer call lights promptly and complete proper documentationProvide escorts for residents to and from meals and activities as neededReport any physical, mental or emotional changes of residents to Nurse., bruises, skin tears, depression, anxiety, lack of appetite, etc.Attend and participate in regularly scheduled staff meetings and trainingsPromote harmony and cooperation between shifts and buildingsResponsible for the transfer and security of all community equipment, including pager, radio, keys, etc,Follow chain of command when reporting concerns or incidentsMust speak English when having direct contact with residents and in all common areasMaintain confidentiality according to HIPAA standardsOther duties as assigned by the Executive DirectorOne great perk of working at Dimensions Living, Appleton is we offer on-demand pay through our financial wellness provider, UKG Wallet.
