How you’ll make an impact in this role - Coordinate and serve as the primary leader of a multidisciplinary care team to ensure well-planned, holistic, and safe patient care. - Implement and continuously monitor individualized patient care plans, driving improved health outcomes and adhering to all established clinical protocols. - Provide compassionate, direct nursing care while serving as the patient’s advocate to uphold the highest standards of quality and safety. - Manage the patient’s status, recording vital information and proactively communicating critical changes to the physician and healthcare team for timely intervention. - Develop and coordinate patient discharge plans, facilitating a successful and supported transition out of the facility and into post-acute care. - Travel to other Urgent Care clinics in the Fox Valley area as needed. What minimum requirements you’ll need Licensure / Certification / Registration: - BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. - Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State. Education: - Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.
