experience in pain and symptom management End-of-life care Empathy
and compassion Description: Home care and hospice for a primarily
geriatric population. Common diagnoses include heart disease,
chronic respiratory diseases, CHF, COPD, pneumonia, dementia,
Alzheimer's, depression, stroke, cancer, kidney disease, diabetes,
liver disease, wounds, joint replacement, surgery, and medication
management. Daily 15-minute huddle call, weekly IDT meeting, and
monthly all-staff meeting are required. Patients will self-manage
CPAP and BiPAP; no vents are used. The RN will see 5-6 patients per
day, with a total of 25-30 visits per week, including routine and
OASIS visits. Administrator is always available after hours, on
weekends, and holidays. Three certified nursing assistants are
available for support. Shifts are Monday through Friday from 8:00
AM to 4:30 PM, with end times that may vary. Weekend work is
required only during on-call shifts. Availability for at least one
major holiday and one minor holiday is necessary. There is a call
requirement of one on-call week/weekend every six weeks; on-call
days do not have to be consecutive. No floating is required, but
both home health and hospice visits will be conducted. Solid color
scrubs are required; no patterns or logos unless company-issued.
Closed-toe, non-slip shoes with socks or hosiery are mandatory. No
flip-flops, open-toe shoes, or sandals are allowed in any patient
care area. All clinicians must clock in and out at the facility. 5
