Job Description:

Ascension seeks a dedicated RN-Patient Navigator Oncology to join our compassionate team committed to holistic patient-centered care in our specialized oncology unit. As part of our faith-based organization, you will play a pivotal role in enhancing patient experiences through innovative care strategies and a consistent, supportive approach, reflecting our mission to serve and uplift the community through personalized healthcare services. Assess and analyze patient needs and healthcare outcomes in oncology. Develop and implement patient-specific care plans in collaboration with multidisciplinary teams. Provide education and resources to patients and their families regarding cancer treatments, side effects, and health maintenance during and post-treatment. Act as a liaison between patients and healthcare providers to ensure clear communication and seamless coordination of care. Monitor patient progress and adjust care plans as necessary. Advocate for patients' needs and preferences with healthcare teams and external resources. Document patient statuses and care provided in electronic health records accurately and comprehensively. Strong interpersonal and communication skills Knowledge of oncology care practices Proficiency in electronic health record systems Critical thinking and problem-solving skills Ability to educate patients and their families on complex medical information Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Valid RN license Oncology Certified Nurse (OCN) preferred Basic Life Support (BLS) Certification
